Healthy Rewards for West Virginia members
Sometimes we all need cheering on
Starting and keeping up with healthy habits can feel like a chore. With Healthy Rewards, you can earn gift cards for the healthy habits you or your child are building. It’s a no-cost, optional program for Wellpoint Medicaid and WVCHIP members.
If you’re a Wellpoint Medicaid or WVCHIP member, you may earn rewards for:
- Completing a new member health screener—$10.
- Childhood and adolescent yearly wellness visit, ages 3–21—$25.
- Adult yearly wellness visit, ages 22–64+ —$25.
- Completing childhood vaccinations series (four (DTaP); three polio (IPV); one (MMR); three (HiB); three (HepB); one chicken pox (VZV); four (PCV); one (HepA); two or three rotavirus (RV); and two (flu) vaccines before members 2nd birthday—$25.
- Completing adolescent vaccinations series (one meningococcal vaccine, one tetanus, diphtheria toxoids and acellular pertussis (Tdap) vaccine, and have completed human papillomavirus (HPV) vaccination series before members 13th birthday — $50.
- Breast cancer screenings every two years ages 40–64+ —$50.
- Cervical cancer screenings ages 21–64—$50.
- Colorectal cancer screenings, ages 45–75 —$25.
- Dental visits for members two and older—$25.
- 7-day follow-up after emergency department visit for a behavioral health condition —$25.
- A diabetic retinal eye exam, ages 18–64+ —$50.
- A diabetic A1C screening, ages 18–64+ —$50.
Taking Care of Baby and Me® Healthy Rewards for Medicaid and WVCHIP members
Wellpoint Medicaid members can earn even more rewards dollars for prenatal and perinatal care. If you’re pregnant, you can:
- Go to one prenatal visit in the first trimester or within 42 days of enrolling with Wellpoint. This visit earns you $25.
- Attend at least one postpartum visit seven to 84 days after delivery. This visit earns you an added $25.
- Go to at least six well-child checkups by 15 months of age. These visits will earn you $25.
