You’ll get a renewal reminder from Wellpoint letting you know it’s time to renew.
2. Complete your renewal application
Complete the entire renewal application and submit any documents requested by New York State.
3. Submit your renewal application
Submit your completed renewal application and documents before your renewal deadline.

Have you moved? Changed jobs? Got a new phone number? Be sure to update your information with New York State. This will ensure you keep your benefits. Report changes by calling (716) 858-8000 for Erie County or (716) 439-7600 for Niagara County, or by contacting your local county Department of Social Services.
