TennCare CHOICES benefits

Wellpoint members in the TennCare CHOICES program receive long-term care services.

Is CHOICES right for you or your loved one?

This Medicaid program is for adults (age 21 and older) with a physical disability and seniors (age 65 and older). CHOICES assists members with daily activities in their home or helps members to work and be actively involved in their community. The program also provides care in a nursing home, if needed.
