Medicare Advantage Special Needs plans
Chronic Conditions Special Needs Plan (C-SNP)
C-SNP plans offer support and care for people living with:

- A cardiovascular disorder
- Chronic heart failure
Most Medicare Advantage C-SNPs include:
- Prescription drug coverage
- $0 or low monthly premiums
- $0 deductibles
- Additional benefits tailored to your healthcare needs
Dual-eligible Special Needs Plan (D-SNP)
D-SNP plans offer help for people eligible for both Medicare and Medicaid:
- For Medicare, you need to be 65 or older or have certain health conditions.
- For Medicaid, you qualify based on income levels instead of age.
Our D-SNP plans may include:
- Vision coverage
- Dental coverage
- Hearing coverage
- Over-the-counter (OTC) health items
