Dual-Eligible Special Needs Plans with Wellpoint

Wellpoint (formerly Amerigroup) offers plans with more benefits than Original Medicare to people who qualify for both Medicare and Medicaid. These plans are called Dual-Eligible Special Needs Plans (D-SNPs).

Cover more of what you need

Eligible for both Medicare and Medicaid? If so, you might qualify for a Dual-Eligible Special Needs Plan (D-SNP). These plans offer additional coverage you won’t receive with Original Medicare, like dental, vision, and prescription drug benefits. These plans can help with:

Preventative care such as physicals and health screenings

Dental, vision and hearing benefits

Rides to health-related appointments

A 24/7 NurseLine

Personal emergency response system (PERS)

Over-the-counter (OTC) health benefit

How to enroll in a Dual-Eligible Special Needs plan

If you qualify for Medicaid and are enrolled in Medicare, you may be able to enroll in or switch dual plans. You can do this once per Special Enrollment Period (SEP), or during the Medicare Annual Enrollment Period (AEP). After you’re enrolled, you don’t have to renew.

Don’t qualify? Consider one of these similar plans.

If you’re not eligible for a D-SNP, we offer other types of Special Needs Plans (SNPs), including Chronic Condition Special Needs Plans (C-SNPs). Or, our Medicare Advantage plans offer similar benefits. You just need to be enrolled in Original Medicare to apply.
