Am I eligible for a C-SNP?


You may be eligible for a Wellpoint C-SNP plan if you have one of the following conditions and live in an area we cover.

Diabetes

Along with vision, hearing, and dental care, C-SNPs often cover:  

 

 Blood glucose monitoring

  Foot care

 

You’ll see a primary doctor and endocrinologist (diabetes specialist) for your care.

Cardiovascular disorders or chronic heart conditions

C-SNPs can cover conditions such as:  

 

Chronic venous thromboembolism (CVT)
 

Chronic heart failure (CHF)
 

Cardiac arrhythmias

Coronary artery disease
 

Peripheral vascular disease

 

You’ll see a primary doctor and cardiologist (heart specialist) for your care.

Chronic lung disorders

This type of C-SNP provides special help for those with lung-related disorders such as:  

 

Chronic obstructive pulmonary disease (COPD)
