Healthy Rewards is a no-cost, optional program for eligible members enrolled in our health plan. The program encourages you to get the care you need to create a healthy lifestyle and then rewards you with gift cards to popular retailers. You may be eligible to earn rewards for some of the activities below:
- Prenatal care visit
- Baby food program
- Breast cancer screening
- Childhood & adolescent wellness visit
- Diabetic A1c screening
- Flu shot
- High blood pressure medication refill
- Lead screening
- Opioid misuse quiz
- Postpartum care visit
- Well-child visits in the first 30 months of life
- Wellness visits for adults with behavioral health or substance use
Enroll and earn rewards
To find out what you’re eligible to earn, enroll in Healthy Rewards by logging into your Wellpoint account and visiting the Benefit Reward Hub. You can also enroll in the program or learn more by calling Healthy Rewards at 888-990-8681 (TTY 711), Monday through Friday from 9 a.m. to 8 p.m. Eastern time.
